Position Summary

Do you have a passion for nursing? We have the position for you! We are seeking a motivated Registered Nurse with a dedication for delivering compassionate patient care to join our team in Primary Care.
Under the general direction of the local Health Service Administrator and with policy direction from the Community Services Manager (Prince George), the position uses independence and initiative to plan, organize, implement and evaluate professional nursing care for clients of all ages with chronic, acute, palliative or rehabilitative health care needs in community settings; services include assessment, direct medical-surgical nursing care, case management context; ensures that the model of client self-care and independence is maintained; participates in the administration of all Acts and Regulations applicable to nursing functions; may supervise ancillary staff. Receive referrals, complete Long Term Care assessments to determine eligibility for service, assign levels of care, establish care requirements and arrange appropriate delivery of care as well as wait listing for facility placement. Works with community groups to develop resources, represents the program at formal and informal settings in the community.
Primary Care Networks are partnerships between the Divisions of Family Practice representing BC's family physicians, the regional health authorities, the First Nations Health Authority (FNHA) and local community partners, along with other community providers such as nurse practitioners. All partners are working together in their local communities and collectively to redesign B.C.'s primary and community care system so that it works better for patients and health-care providers.
Shift Rotation/Hours of work: Monday to Friday, Days, 08:30 to 16:30

Qualfications
• Registration with BC College of Nurses and Midwives as a practicing RN registrant.
• 2 years' experience in acute care and / or community health nursing or a diploma in nursing.
• Plus three (3) years' experience in home care or acute care nursing.
• Diabetes Educator Course.
• CPR Certification.
Skills and Abilities:
• Knowledge of and skill in acute nursing and community nursing. Gerontologic nursing experience preferred.
• Ability to adapt clinical and professional skills to home and community settings.
• Considerable judgment in professional nursing care.
• Ability to establish and maintain effective working relationships with all members of the health care team.
• Ability to communicate and maintain good public relations with family and friends of patients.
• Must be able to work independently and with minimal supervision.
• Ability to communicate effectively, verbally and in writing.
• Ability to promote safe, competent, patient care. Ability to organize and set priorities.
• Problem solving skills. Program planning, evaluation, counseling and teaching skills.
• Valid driver's license.
